Co-op Procedure & Forms

Please fill in the Information Collection form about your upcoming plan and Co-op intention.

If you would like the School to help with the Co-op placement, please fill in the Collection Form for Co-op Opportunity📝: Fill this form ONLY IF you need us to help you with Co-op placement.
Please fill in the Work Experience Agreement & Training Plan📝and return it to us before the Co-op starts, or no later than one month after the Co-op starts. This form needs to be signed by the employer/hiring manager.
Please fill in the Work Experience Joint Evaluation Form📝once your Co-op is completed. This form needs to be completed and signed by the employer/hiring manager. If you plan to have multiple jobs or experience job changes, you are required to fill out the Work Experience Agreement & Training Plan📝 and the Work Experience Joint Evaluation Form📝 for each employment.
